New Patient – Pediatric (2-12 years)
For children ages 2-12 who are new to our office or have not been seen in over 3 years.
New Patient – Infant (0-2 years)
For infants and toddlers who are new to our office or have not been seen in over 3 years.
Returning (Been A While) Patient – 1 to 3 Years
For patients who have not visited us in 1-3 years.
Returning (Reconnect) Patient – 6 to 12 Months
For patients who have not visited us in 6-12 months.
